  Objective: This retrospective study aimed to investigate the frequency, causes, and temporal distribution of orthodontic emergencies at the Department of Orthodontics, University of Oslo. Methods: All appointments coded as emergencies from 2015 to 2022 were reviewed and manually screened. Emergencies were classified into three categories: (1) during active treatment, (2) during retention, and (3) miscellaneous. A negative binomial generalized linear regression model was used to assess annual and monthly variations. Chi-square tests for independence were applied to analyze relationships between age group and year, gender and year, and reason for emergency. The significance level was set at p < 0.05. Results: Out of 7206 records screened, 3592 met the inclusion criteria, involving 1657 patients – 685 males (41.3%) and 972 females (58.7%). The mean number of emergencies per patient was 2.2, with no significant year-to-year variation in gender distribution. The most common age groups were 10–14 years and over 20 years. Annual emergency numbers ranged from 313 in 2015 to 638 in 2022, with statistically significant variation. Monthly variation was also significant, with July consistently having the fewest emergencies (p < 0.001). Most emergencies occurred during active treatment (n = 2021), followed by the retention phase (n = 1353), and miscellaneous reasons (n = 218). The most frequent causes were issues related to full fixed appliances and fixed retainers. Conclusions: No consistent pattern of orthodontic emergencies was observed, indicating that operator experience, the stage of postgraduate training, and seasonal fluctuations are likely the key influencing factors

  Tooth autotransplantation is a biologically based and cost-effective surgical procedure in which a tooth is repositioned within the same individual, offering a valuable alternative to prosthetics or implants, particularly in growing patients. Modern protocols, developed through advances in surgical techniques, imaging, and case selection, have significantly improved predictability and success rates. This narrative review aims to synthesize current knowledge on periodontal and pulpal healing following autotransplantation, with emphasis on biological mechanisms and factors influencing clinical outcomes. Periodontal healing depends largely on preservation of periodontal ligament vitality and stem cells, enabling regeneration and functional integration. Revascularization, increased dentin formation and obliteration of the pulp are favourable for survival of the transplant, although complete histological restoration is rare. In general, immature teeth demonstrate superior regenerative capacity compared to mature teeth, but complications such as root resorption and ankylosis remain challenges in both instances. Radiographic evaluation provides limited insight during early healing stages, highlighting discrepancies with histological findings. In conclusion, successful autotransplantation relies on careful case selection, atraumatic technique, and biological preservation, though further human-based research is needed to clarify healing mechanisms and optimize long-term results.

  Objectives: The aim was to analyze current orthodontic retention protocols in Croatia and their trend over a 10-year period. Subjects and Methods: In a repeated cross-sectional survey, questionnaires were distributed to 150 orthodontists in Croatia in 2013 and 185 of them in 2023 (representing 69% and 85% of practicing orthodontists with response rates of 61% and 75%). Results: Significant changes in retainer types across different malocclusions and treatment protocols were observed over a 10-year period. There was an increase in the use of VFR and a decrease in acrylic retention plates in both jaws (p≤0.001). Over the decade, the use of only fixed retention in the mandible declined from 20% to 7% (p=0.007), and the combination of fixed and removable retention in the maxilla dropped from 29% to 17% (p≤0.033). Increased use of removable retainers was noted in cases involving rotations, open bites, and extractions. Conversely, fixed-removable retainers were more frequently used in spacing cases (p≤0.001). There was a trend toward fewer check-ups (p=0.001) and toward increased patient self-monitoring (p=0.021). Personal experience as the primary reason for choosing a retention protocol increased from 39% to 62% (p<0.001). Additionally, the use of written information on retention rose from 39% to 69% (p<0.001). Conclusion: Retention practices in Croatia have evolved over the past decade, characterized by increased use of VFRs, more frequent implementation of dual retention in the mandible, and greater provision of written retention information. Additionally, follow-up protocols have shifted toward fewer scheduled visits, with increased emphasis on patient self-monitoring.

  Abstract Objectives Evaluate long‐term spontaneous occlusal changes following L7 extraction in adolescent patients. Materials and Methods Study models of 144 participants (63 males, 81 females) retrospectively assessed prior to L7 extraction (9–16 years old; T1) and following L8 eruption (14–25 years old; T2). All received upper fixed appliances. A sub‐group (n = 86) received no lower fixed appliances and acted as controls. Occlusal changes were compared between treatment (lower fixed appliance) and control (no lower fixed appliance) groups using PAR index. At T2, L8 occlusal outcome was assessed using ABO grading system. Results Mean follow‐up period 6 (SD 2) years. At T1, lower scores observed in control group for Lower Anterior ( P  &lt; .001), Midline ( P  = .033) and Lateral Segments ( P  = .040) components. At T2, lower scores continued being observed in control group for Midline ( P  &lt; .001) and Lateral segment ( P  = .019) components. Higher decrease in Lower Anterior PAR scores observed in treatment group (&lt;.001) with comparable scores between groups at T2 ( P  = .057). Similar PAR score changes between groups for Lateral Segments, Overjet and Overbite components. At T2, no significant difference observed in Total PAR score reduction between control (83%) and treatment (82%) groups. Good‐to‐acceptable occlusal outcome of the L8 observed in 81.55% of cases at T2 with no difference between groups. Conclusion In growing patients with mild mandibular crowding, extraction of L7 followed by upper fixed appliance therapy, leads to favourable occlusal changes over a 6‐year follow‐up period, with or without lower fixed appliance therapy, being an alternative extraction protocol where lower fixed appliance therapy is not recommended.

  With the help of educators from all over Europe, the European Guidelines for Postgraduate Education in Orthodontics were created, and its influence spread far and wide. These recommendations, which promote evidence-based treatment and integrate the most recent developments in orthodontic research and technology, have enabled orthodontic instructors and postgraduate students globally to stay up to date with the quickly changing field of orthodontics. The World Federation of Orthodontists (WFO) Guidelines for Postgraduate Orthodontic Education, which were released in 2009 and later revised in 2023, were also influenced by the European Guidelines. The use of these guidelines by postgraduate program directors around the world and by related educational, scientific, and administrative institutions will allow them to adapt their curricula to worldwide standard and to strive for excellence in themselves and our speciality.

  Background Several appliances or treatment protocols are marketed to either patients or orthodontists as being associated with improved orthodontic outcomes. However, clinical decision-making should be based on robust scientific evidence and not marketing claims or anecdotal evidence. Objective To identify appliances/protocols being associated with improved outcomes of fixed appliance treatment. Search methods Unrestricted literature searches in seven databases/registers for human studies until March 2024. Selection criteria Randomized or quasi-randomized clinical trials on human patients of any age, sex, or ethnicity receiving comprehensive orthodontic treatment with fixed appliances and assessing occlusal outcome with either the Peer Assessment Rating (PAR) or the American Board of Orthodontics-Objective Grading System (ABO-OGS) index. Data collection and analysis Duplicate/independent study selection, data extraction, and risk of bias assessment with the Cochrane RoB 2 tool. Random-effects meta-analyses of averages or mean differences with their 95% Confidence Intervals (CI), followed by meta-regression/subgroup/sensitivity analyses and assessment of the quality of clinical recommendations with the Grades of Recommendations, Assessment, Development, and Evaluation (GRADE) approach. Results Data from 20 small- to moderately-sized trials covering 1470 patients indicated that orthodontic treatment with fixed appliances is effective and results on average in a final PAR score of 6.0 points (95% CI 3.9–8.2 points), an absolute PAR reduction of 23.0 points (95% CI 15.6–30.4 points), a % PAR reduction of 82.6% (95% CI 70.8%–94.4%), and an absolute ABO-OGS score of 18.9 points (95% CI 11.7–26.2 points). However, very high between-study heterogeneity (I2 > 75%) was seen for both PAR and ABO-OGS. Extraction treatment was associated with significantly better occlusal outcome than non-extraction treatment with ABO-OGS (12.9 versus 16.6 points; P = .02). There was no statistically significant difference in occlusal outcome with (i) 0.018″-slot or 0.022″-slot brackets; (ii) customized or prefabricated brackets; (iii) anchorage reinforcement with temporary anchorage devices; (iv) use of vibrational adjuncts; and (v) aligners or fixed appliances (P > .05 in all instances), while small benefits were seen with indirectly bonded brackets. Conclusions Considerable between-study heterogeneity exists in the reported occlusal outcome of fixed appliance treatment, and different appliances or adjuncts have little effect on this. Standardization and/or automatization of the scoring procedures for PAR and ABO-OGS might help to improve consistency and reliability of outcome measurement in orthodontic trials.

  Background CAD/CAM (computer-aided design/computer-aided manufacturing) fixed retainers (FRs) as an alternative to multistranded FRs to maintain orthodontic treatment outcome. Objectives The primary aim was to compare CAD/CAM versus conventional multistranded FRs in terms of stability until 2 years. Secondary outcomes were failure rates, patient satisfaction, and cost-minimization. Trial design 2-arm parallel, two-centre randomized controlled trial. Methods Patients were randomized to CAD/CAM or conventional FRs in both arches, in a 1:1 ratio and blocks of four. Allocation concealment was secured by using sequentially numbered envelopes. Patients were blinded. FRs were bonded at the end of treatment, and patients were recalled after 12 and 24 months. First-time retainer failures were recorded and digital impressions were taken. Arch widths and lengths, as well as Little’s Irregularity Index (LII), were measured. Additionally, patients answered satisfaction questionnaires. Linear mixed models were applied for measurements and patient satisfaction. Survival analyses were estimated with Kaplan–Meier curves, along with Cox-regression modelling. Cost-minimization analysis was undertaken. Results One hundred and eighty-one patients were randomized (98 in Centre 1, and 83 in Centre 2): 90 in CAD/CAM and 91 in conventional group. One hundred and fifty three patients attended T24 follow-up. There were no significant differences in LII and arch dimensions between groups for failure-free patients. Within 24 months, 34% maxillary CAD/CAM FRs and 38% maxillary conventional FRs failed, along with 42% mandibular CAD/CAM FRs and 40% mandibular conventional FRs, with no significant difference in survival between groups (hazard ratios conventional to CAD/CAM: maxillary arch: 1.20 [P = 0.46], mandibular arch: 0.98 [P = 0.94]). There were no significant differences in patient satisfaction between groups. No harms were observed. Cost-minimization analysis showed that CAD/CAM FRs were slightly cheaper than conventional FRs. Conclusions There were no clinically significant differences in LII, arch widths, and lengths between CAD/CAM and conventional FRs after 24 months. There were no differences in failures and patient satisfaction between groups. CAD/CAM FRs were slightly cheaper than conventional FRs.

Results One hundred and eighty-one patients were randomized (98 in Centre 1, and 83 in Centre 2): Ninety in the CAD/CAM group and 91 in the conventional group. Three subjects dropped out at baseline, as they did not attend any of the follow-up appointments.168 patients attended the T6 visit. There were no significant differences in arch dimensions between T0 and T6, whilst the LII was different only in the CAD/CAM group (mean difference: 0.2 mm; 95% confidence interval: 0.1 to 0.4; P &amp;lt; 0.001). Within 6 months, 39 upper retainers (19 out of 88 CAD/CAM and 20 out of 90 conventional retainers) and 52 lower retainers failed (26 out of 88 CAD/CAM and 26 out of 90 conventional retainers), with no significant difference between the survival of both types of retainers (hazard ratios conventional to CAD/CAM: upper arch: 0.99 [P =0.99], lower arch: 0.93 [P = 0.80]). There were no significant changes in patient satisfaction between the groups. No harms were observed. Conclusions There were no clinically significant differences in LII, arch widths and lengths between CAD/CAM and conventional retainers after 6 months.   Background Orthodontic retention is the most important factor after successful orthodontic treatment. The use of thermoplastic retainers has increased in recent years, but information is lacking about the product materials and orthodontists’ awareness of the products they use. The aim of this survey was to map the retention protocols among Scandinavian orthodontists, particularly their use of thermoplastic retainers. Furthermore, the aim was to investigate their knowledge of thermoplastic materials and record any possible adverse effects. Methods An online questionnaire was prepared, and 667 orthodontists in Norway, Sweden, and Denmark were invited to take the survey. The survey was sent to all members of the national orthodontic associations using Nettskjema in Norway and Microsoft Forms in Sweden and Denmark. Data were collected anonymously and analyzed using chi-square and correlation coefficients. Results Of the 667 orthodontists, 432 (64%) responded (59% female). The most common retention protocol (51%) was fixed retainer in both maxilla and mandible and thermoplastic retainer in the maxilla. Two-thirds of the orthodontists were unaware of the thermoplastic material used, and 58% did not acquire knowledge of the materials. Only 1% of the respondents had registered adverse reactions to thermoplastic retainers, and none were aware of the type of material that was used. Conclusions Scandinavian orthodontists use similar retention protocols, with the most common being fixed retainer in the mandible and dual retention, fixed, and thermoplastic retainer in the maxilla. Orthodontists’ knowledge about thermoplastic materials was insufficient, but adverse effects related to thermoplastic retainer use were rare.

  Objective This randomized controlled trial (RCT) aims to investigate the short-term effects of chlorhexidine mouthwash (MW) on gingival health surrounding orthodontic miniscrew implants (OMIs) and their overall survivability. Materials and Methods Thirty-two participants (mean age, 22.8 years) undergoing fixed orthodontic appliance treatment after maxillary premolar extraction were randomly allocated in a parallel fashion to either receive (1) MW with an active component of chlorhexidine or (2) a placebo. Each participant received two maxillary buccal OMIs for anchorage reinforcement purposes. Participants were assessed for their gingival oral health status around all inserted OMIs and had their OMI survivability recorded at three time points; T1 = 1 month, T2 = 3 months, and T3 = 6 months after OMI placement. A Kaplan-Meier plot was used to estimate the survival function of OMIs. Results All randomized participants completed the follow-up period. In terms of gingival oral health, there were no statistically significant differences at any time point between the chlorhexidine MW group and the placebo-controlled group (P > .05). One OMI was lost in the chlorhexidine MW group and another two OMIs in the control group. There was no significant difference between both groups in terms of survivability (P = .585). Conclusion The use of chlorhexidine MW does not seem to have a significant clinical impact on gingival health around OMIs or their survivability in this pilot study.

  Background: This cross-sectional study evaluated patient-reported outcome measures (PROMs) on (1) oral hygiene, (2) periodontal health, (3) retainer failure, (4) orthodontic treatment satisfaction, and (5) outcome satisfaction in orthodontic retention patients. The purpose of the study was to evaluate whether orthodontic retention treatment is associated with patient-reported outcome measures on oral hygiene, periodontal health, and treatment satisfaction. Methods: A ten-item questionnaire on the five PROMs was conducted among 211 consecutive retention patients up to ten years following orthodontic treatment. Linear regression models were computed to detect possible associations between the PROMs and retention treatment or patient characteristics. Results: The presence of a fixed lingual retainer was not associated with the reduced ability to perform oral hygiene, self-perceived periodontal health, or orthodontic outcome satisfaction. Older patients were more content with the orthodontic treatment result (p &lt; 0.05). Patients with fixed lingual retainers in the mandible were less satisfied with the course of orthodontic treatment (p &lt; 0.05). Smokers more often reported gingival bleeding (p &lt; 0.05). Females reported increased gingival recessions (p &lt; 0.05) and perceived their teeth as longer than before treatment (p &lt; 0.05). Longer orthodontic treatment duration corresponded to retainer failure (p &lt; 0.05). Conclusions: In general, long-term orthodontic retention patients were satisfied with orthodontic treatment. These patients reported the satisfactory ability to perform adequate oral hygiene and periodontal health, and they communicated a high degree of treatment and outcome contentment. However, patients with a retainer in the mandible were less satisfied with orthodontic treatment.

  The objective of this study was to investigate the relationship between the absence, presence and dynamics of mandibular third molar development and the occurrence and amount of late mandibular incisor crowding. Dental plaster casts and panoramic radiographs of 72 orthodontically untreated subjects from the Nittedal growth study, Norway were analyzed. The subjects were recalled for a checkup at 12, 15, 18 and 21 years of age. Mandibular incisor crowding was assessed using Little’s irregularity index and dental maturation of the third molars by the Cameriere’s index. The majority of the subjects (64%) had ≥1 mm increase in irregularity; 22% experienced an increase of 0.1–0.9 mm and 14% had unchanged or decreased irregularity. Incisor irregularity increased with age, regardless of absence or presence of third molars. The amount of change in incisor irregularity from 12 to 21 years did not differ significantly between subjects with hypodontia of third molars, extraction and those with third molars present. No differences were observed between erupted, unerupted or impacted third molars. No correlation was found between the amount of change in irregularity and maturation of the third molars. In conclusion, occurrence and amount of mandibular late incisor crowding is not significantly influenced by the presence of mandibular third molars or their development dynamics.

  Abstract Purpose Orthodontic fixed appliances have been proven to be effective in treating a wide variety of malocclusions, and different types of appliances have emerged during recent decades. However, the comparative effects of different appliances have not been adequately assessed. Thus, the aim was to assess the occlusal outcome of orthodontic treatment with preadjusted straight-wire (SWIRE) and standard edgewise (SEDGE) appliances. Methods In all, 56 patients (mean age: 13.5 years; 45% male) receiving extraction-based treatment with either SWIRE or SEDGE appliances were included. Between-group differences in the occlusal outcome assessed with the American Board of Orthodontists Objective Grading System (ABO-OGS) and treatment duration were analyzed statistically at the 5% level. Results The average ABO-OGS score was 31.3 ± 7.2 points and 34.0 ± 10.4 points in the SWIRE and SEDGE groups with no statistically significant difference between groups ( P  = 0.26). Treatment duration was significantly shorter in the SWIRE group compared to the SEDGE group, with an average difference of −6.8 months (95% confidence interval [95% CI] = −9.6 to −4.0 months; P  &lt; 0.001). Likewise, fewer visits were needed with SWIRE compared to SEDGE appliances with an average difference of −7.2 visits (95% CI = −10.3 to −4.2 visits; P  &lt; 0.001). Adjusting for the influence of any potential confounders did not considerably impact the results. Conclusion Similar treatment outcomes were observed after premolar extraction treatment with SWIRE and SEDGE appliances. On the other hand, SEDGE appliances were associated with prolonged treatment duration and more visits needed to complete treatment compared to SWIRE appliances.

  Background Even though treatment of Class II malocclusion with premolar extractions and incisor retraction might affect incisor inclination and soft tissue profile, the effects of bracket prescription on this have not been thoroughly assessed. Methods Fifty patients (mean age: 13.6 years; 34% male) receiving extraction-based treatment with either standard Edgewise or pre-adjusted appliances were included. Between-group differences in the incisor inclination assessed with lateral cephalograms were analyzed statistically with linear/logistic regression at 5%. Results Treatment-induced changes included retroclination of the upper/lower incisors (−3.0° and −2.0°, respectively), retraction of the upper/lower incisors (−3.4 mm and −1.5 mm, respectively), retraction of the upper/lower lip (−2.1 mm and −2.0 mm, respectively), and enlargement of the nasolabial angle (+1.6°). Analysis of the data adjusting for confounders indicated that the pre-adjusted group, after treatment, had larger inclination of the upper or lower incisors (+3.2° and +4.5°, respectively), more prominent upper incisors relative to the facial plane (+1.3 mm), and smaller interincisal angle (−7.3 or −7.7°). Post-treatment upper incisor inclination fell within the cephalometric norm significantly more in the pre-adjusted than in the standard Edgewise group (odds ratio 4.3; 95% confidence interval 1.1–16.6). No differences were found in lower incisor prominence, upper/lower lip prominence, or nasolabial angle. Conclusions Pre-adjusted appliances were associated with increased inclination of the upper and lower incisors, with more prominent upper incisors, and with more acute interincisal angle after retraction compared with standard Edgewise appliances. However, such differences did not translate in greater retraction of the upper/lower lips and greater nasolabial angle.

  Objective To assess periodontal status in long-term orthodontic retention patients and investigate possible risk indicators. Materials and Methods Plaque index (PI), gingival index (GI), probing pocket depth (PPD), gingival recessions (GR) and calculus were recorded in 211 patients with or without fixed retainers. Results Periodontal parameters were within the limits of clinically healthy periodontium. The use of fixed retainers was associated with higher PI in the maxilla (ß = 1.10 [0.37]; p <.05). Older age was associated with higher PI in the mandible (ß = 0.27 [0.11]; p <.05). Smoking was associated with gingival bleeding on both palatal (ß = 0.63 [0.16]; p < .01) and labial sides in the maxilla (ß = 0.46 [0.20]; p <.05). Smoking was also associated with increased prevalence of GR in the mandible (ß = 0.24 [0.07]; p <.01), while use of snuff had similar effect on the labial side in the maxilla (ß = 0.35 [0.08]; p <.01). Higher age (ß = 0.05 [0.02]; p <.05) and the presence of a retainer (ß = 0.23 [0.07]; p < .05) were associated with calculus accumulation on the lingual side in the mandible. Conclusion The present observational study suggest that long-term fixed retainers alone have no detrimental effect on the periodontium. Additional factors may increase the risk of plaque deposits and increased probing pocket depths. Further prosepective studies are needed to confirm the present outcome.

  Objective The objective of the study was to examine the effect of irisin on human periodontal ligament cells (hPDLCs) growth, migration and osteogenic behaviour in vitro. Materials and methods Primary hPDLCs and human osteoblasts (hOBs), used as positive controls, were cultured with irisin (10 and 100 ng/ml), and effect on cell proliferation was evaluated with 5-bromo-2`-deoxyuridine incorporation at 1, 2, and 3 days, and on migration capacity was investigated by scratch assay at 2, 6, and 24 h. Osteogenic behaviour was assessed with alkaline phosphatase activity, immunoassay at 3, 7, 14, and 21 days, and confocal laser scanning microscopy at 21 days. Mineralization was examined by Alizarin red staining at 21 days. Data were compared group wise using ANOVA tests. Results Irisin induced increased proliferation of primary hPDLCs and hOBs at all time points compared to untreated controls. This was confirmed by scratch assay where irisin enhanced migration of both hPDLCs and hOBs after 6 and 24 h compared to controls. Irisin treatment promoted osteogenic behaviour of both cell types by enhancement of extracellular matrix formation. In hPDLCs irisin increased expression of type I collagen, secretion of osteoblastogenesis related proteins osteocalcin and leptin, and calcium deposition/mineralization compared to controls at 21 days. In addition, to enhance calcium deposition/mineralization in hOBs, irisin increased expression of periostin, and secretion of osteoblastogenesis related proteins osteopontin, alkaline phosphatase and osteocalcin, as compared to controls at 21 days. Conclusions Primary hPDLCs responded to irisin treatment with enhanced cell growth, migration, and matrix formation in vitro.

  Background: Torque of the maxillary incisors is crucial to occlusal relationship and esthetics and can be influenced by many factors. The aim of this study was to assess the relative influence of the material of the orthodontic appliance (adhesive, bracket, ligature, and wire) on tooth displacements and developed stresses/strains after torque application. Methods: A three-dimensional upper right central incisor with its periodontal ligament (PDL) and alveolus was modeled. A 0.018-in. slot discovery® (Dentaurum, Ispringen, Germany) bracket with a rectangular 0.018 x 0.025-in. wire was generated. The orthodontic appliance varied in the material of its components: adhesive (composite resin or resin-modified glass ionomer cement), bracket (titanium, steel, or ceramic), wire (beta-titanium or steel), and ligature (elastomeric or steel). A total of 24 models were generated, and a palatal root torque of 5° was applied. Afterwards, crown and apex displacement, strains in the PDL, and stresses in the bracket were calculated and analyzed. Results: The labial crown displacement and the palatal root displacement of the tooth were mainly influenced by the material of the wire (up to 150% variation), followed by the material of the bracket (up to 19% variation). The magnitude of strains developed in the PDL was primarily influenced by the material of the wire (up to 127% variation), followed by the material of the bracket (up to 30% variation) and the ligature (up to 13% variation). Finally, stresses developed at the bracket were mainly influenced by the material of the wire (up to 118% variation) and the bracket (up to 59% variation). Conclusions: The material properties of the orthodontic appliance and all its components should be considered during torque application. However, these in silico results need to be validated in vivo before they can be clinically extrapolated.

  Objective The objective was to examine the effects of repetitive local administration of adiponectin on experimental tooth movement in rats. Materials and methods The maxillary right first molar of male Wistar rats (n = 24) was moved mesially for 14 days, with local adiponectin injections (0.2 or 2 μg) every third day. Micro-computed tomography was performed at days 0, 6 and 14 and molar movement, bone density and bone volume fraction were calculated from the scans. Changes in extracellular matrix collagen and cell numbers in the periodontal ligament were analyzed histologically, and levels of circulating cytokines were measured by Luminex and ELISA. Results Adiponectin injections induced a reduction in tooth movement after 12 and 14 days compared to controls. No tooth movement was observed between days 3 and 14 in the group receiving the highest dosage (2 μg) of adiponectin. Differences in bone density and bone volume fractions between treatment and control groups were not identified. Relative size and morphology of collagen fibrils, and cell number in the periodontal region after adiponectin injections were unchanged compared to controls. Levels of circulating adiponectin or other selected factors in plasma were not influenced by the adiponectin injections. Conclusions Submucosal injections of adiponectin prevented experimental tooth movement in rats. The effect was dosage-dependent and local. Adiponectin injections caused no detectable changes in bone density, periodontal cell number or collagen content.

  The purpose was to compare the treatment effects of functional appliances activator-headgear (AH) and Twin Block (TB) on skeletal, dental, and soft-tissue structures in class II division 1 malocclusion with normal growth changes in untreated subjects. The sample included 50 subjects (56% females) aged 8–13 years with class II division 1 malocclusion treated with either AH () or TB () appliances. Pre- and posttreatment lateral cephalograms were evaluated and compared to 50 untreated class II division 1 cases matched by age, gender, ANB angle, and skeletal maturity. A paired sample, independent samples tests and discriminant analysis were performed for intra- and intergroup analysis. Treatment with both appliances resulted in significant reduction of skeletal and soft-tissue facial convexity, the overjet, and the prominence of the upper lip in comparison to untreated individuals (). Retroclination of maxillary incisors and proclination of mandibular incisors were seen, the latter being significantly more evident in the TB group (). Increase of effective mandibular length was more pronounced in the TB group. In conclusion, both AH and TB appliances contributed successfully to the correction of class II division 1 malocclusion when compared to the untreated subjects with predominantly dentoalveolar changes.

  Objective: To investigate the long-term influence of fixed lingual retainers on the development of mandibular gingival recession and to compare the prevalence with untreated individuals. Materials and Methods: The material consisted of 144 subjects: 96 orthodontically treated patients followed for 5 years after therapy and 48 untreated age-matched subjects. The treated patients were divided in two groups: one receiving a fixed mandibular retainer (n = 48) and one receiving no form of retention in the mandible (n = 48). The presence or absence of gingival recession and calculus accumulation were scored before treatment (T0), after debonding (T1), and 5 years after debonding (T5) for each tooth in the mandibular intercanine region using plaster models and intraoral photographs. The chi-square test, one-way ANOVA, and Cochran's Q test were used to evaluate inter- and intragroup differences. Results: The prevalence of patients with recession increased gradually and significantly throughout the observation periods in all groups, but the intergroup differences at T5 were not significant. Significantly more calculus accumulation was observed at T5 in the retainer group compared with the group without retainers. Conclusions: Long-term presence of fixed lingual retainers does not seem to increase the development of mandibular gingival recession, but does increase calculus accumulation. © 2017 by The EH Angle Education and Research Foundation, Inc.

  Structured Abstract Authors - Franzen T.J., Monjo M., Rubert M., Vandevska-Radunovic V. Objectives - To investigate biological changes of alveolar bone occurring during orthodontic relapse. Materials and Methods - Maxillary first rat molars were moved mesially for ten days. After orthodontic tooth movement (OTM) appliances were removed, and the molars were allowed to relapse for one, three, five, seven, 14 or 21 days. Changes in 3D morphometric parameters of bone located mesial to the first molars were evaluated by micro-CT. Total RNA was isolated from the same bone site and real-time RT-PCR was used to measure the expression of bone formation and resorption markers. Results - One day after appliance removal, the molars relapsed to a mean 73% of the achieved OTM, and then steadily relapsed to 93% at 21 days. Tissue mineral density and percent bone volume increased over the experimental period. Inversely, there was a decrease in total porosity. Gene expression of OCN, Coll-I and ALP decreased during OTM, and as the molars relapsed showed an increasing tendency. Gene expression of RANKL and TRAP increased during OTM. Changes in mRNA expression of H+-ATPase were minor. By 21 days post-appliance removal the remodelling process in rats appeared to have returned to control levels. Conclusions –Bone tissue reactions on a molecular level are similar during OTM and orthodontic relapse. These findings validate the importance of immediate retention following active OTM.   Objectives:The purpose of this study was to investigate time-related dental tissue reactions to tooth replantation in mature and immature human premolars by histomorphometric and radiographic methods.Material and methods:Sixty premolars were extracted, immediately replanted (T0), and left without intervention for 1, 2, 3, 6, 12, and 24 weeks before re-extraction (T1). Periapical radiographs were taken at T0 and T1 in all groups except for the 24 week group. Paraffin-embedded teeth were sectioned buccolingually at 5 µm, stained with haematoxylin-eosin, and prepared for histological analysis. Root resorption, tertiary dentine, and root and crown length were recorded and compared on both radiographs and paraffin sections.Results:Initial degeneration of the odontoblast layer and disturbance of vasculature and normal architecture of the pulp were seen in both mature and immature teeth within the first 3 weeks. Revascularization and tertiary dentine formation was visible on histological sections in immature teeth at 6 weeks. After 24 weeks, most immature teeth had pulps partly or completely obliterated. Abundant tertiary dentine observed on histological slides was not easily seen on periapical radiographs. Likewise, surface root resorption found in both immature and mature teeth was not visible on the radiographs up to 6 weeks after replantation.Conclusion:Within the limits of this study it can be concluded that the dental pulp of immature teeth has the ability to regenerate and produce tertiary dentine after replantation. Root resorptions and tertiary dentine seen on histological sections are not visible on corresponding radiographs 6 and 12 weeks after replantation.

  Relapse after orthodontic tooth movement (OTM) is an undesirable outcome that involves a number of factors. This study investigated the remodelling of the alveolar bone and related periodontal structures during orthodontic relapse in rat molars. The maxillary right first molars of 35 Wistar rats were moved mesially by a fixed orthodontic appliance for 10 days and the contralateral molars served as controls. The appliances were removed and six animals killed. The molars were allowed to relapse, and the remaining animals were sacrificed at 1, 3, 5, 7, 14, and 21 days. The jaws were sectioned and stained with haematoxylin and eosin and tartrate-resistant acid phosphatase (TRAP). One day after appliance removal, the molars relapsed to a mean 62.5 per cent of the achieved OTM and then steadily relapsed to 86.1 per cent at 21 days. The number of osteoclasts situated along the alveolar bone of the first molars was highest at the end of active treatment and significantly decreased during the relapse period. In the OTM group, osteoclasts were most numerous in the pressure side of the periodontal ligament (PDL). As the molars relapsed over time, the osteoclast distribution shifted, and after 7 days of relapse, TRAP-positive cells were registered in previous pressure and tension sides of the first molars. After 21 days, these cells were concentrated in the distal parts of the PDL of all three maxillary right molars. These results indicate that orthodontic relapse in the rat model occurs rapidly and remodelling of the alveolar bone and PDL plays a central role in the relapse processes of both actively moved and adjacent teeth.

  Aims: To survey retention protocols and need for practical retention guidelines among orthodontists in Norway and to compare the results with similar studies in other countries. Methods: A questionnaire was mailed to all members of
the Norwegian Association of Orthodontists. It included questions about their background and their current retention protocol, as well as their perceived need for common retention guidelines. Results: The response rate was 77.7% (69.3% males and 30.7% females). The most common maxillary retainer was a combination of a fixed and removable retainer, followed by a clear thermoplastic retainer. In the mandible, a fixed retainer bonded to all anterior teeth was most common (66.4%). Retention in the maxilla lasted 2 to 3 years (34.7%) or 3 to 5 years (23.8%). In the mandible, 41.5% of the orthodontists left the retainer in place for >5 years. When retention lasted more than 3 years, 70% of the orthodontists left the responsibility for retainer checkups to the patients or the general practitioners. The main reason for choosing a certain retention protocol was clinical experience (57.4%). Only 3.5% of the orthodontists based their protocols on information from the literature. Half
of the orthodontists, significantly women, expressed a need for common retention guidelines. Conclusions: In Norway, bonded retainers alone were reported to be most commonly used in the mandible, while bonded retainers used in combination with a removable retainer appear to be the most commonly used appliances in the maxilla. This is similar to the most frequently used retainers in other countries, but there are disparities in duration and follow-up protocols. Most female orthodontists desire common retention guidelines.   The purpose of the study was to investigate the relationship between root resorption and repair in human premolars that had been orthodontically intruded. The objective was to examine these processes related to time and root development. Seventy-six premolars were divided into subgroups: 33 teeth were intruded and then extracted (G1); 25 teeth were intruded and then left in situ for varying periods before extraction (G2); 18 teeth served as the controls (G3). All teeth were examined by light microscopy. Using non-parametric statistical analysis, differences between the groups were examined with the Pearson chi-square test. Teeth in G1 and G2 had significantly more resorptive lesions, 55 and 64 per cent, respectively, than the controls of 11 per cent. Resorption was observed over the whole root surface and increased with time. The occurrence increased to 100 per cent in both experimental groups after 36 days of intrusion. The appearance of lesions in relation to root development showed no differences between G1 and G2. In the apical part of the root, total resorption of the dentine was sometimes observed, but no resorptions extended into the predentine. Resorptive lesions undergoing repair were seen in both groups, with significantly more repair in G2 (58 per cent) than in G1 (32 per cent). Active resorption and repair were sometimes seen at the same resorption site. Deposition of cellular and acellular cementum was found to the same extent over the whole root when repair took place. With time, resorption appeared over the whole root surface. In some teeth, resorptive activity continued up to 10 days after removal of forces but on the other hand, repair of the resorbed area sometimes started during active movement. The individual variation in repair was much wider compared with resorption. The predentine layer in the apical area appeared not to be affected by the resorptive process.

  Aim: To examine the biological changes occurring in alveolar bone during orthodontic relapse. Materials and Method: Maxillary first rat molars were moved mesially for ten days, appliances were removed, and the molars allowed to relapse for 1, 3, 5, 7, 14 or 21 days. Maxillae of 35 rats were sectioned and stained with H&E and TRAP, whilst in 37 rats 3D morphometric parameters of bone sited mesial to the first molars were assessed by micro-CT, subsequently total RNA was isolated and real-time RT-PCR used to measure the expression of bone formation and resorption markers. Comparison among groups was assessed by t-tests or ANOVA, and correlations by Spearman’s rank correlation coefficient. Results: One day after appliance removal, the molars relapsed to 68.5% of the achieved OTM, and progressively relapsed to 90.6% at 21 days. Osteoclasts located along the alveolar bone of the first molars peaked at the end of active orthodontic tooth movement (OTM) and were most abundant in the pressure side of the PDL. Osteoclast numbers decreased significantly during relapse and their distribution shifted; after 7 days of relapse TRAP-positive cells were registered in previous pressure and tension sides of the first molars. After 21 days, these cells were concentrated in the distal sides of the PDL. In the bone mesial to the first molars tissue mineral density and percent bone volume increased whilst total porosity decreased over the experimental period. Gene expression of OCN, Coll-I and ALP decreased during OTM, and tended to increase as the molars relapsed. Gene expression of RANKL and TRAP increased during OTM. Changes in mRNA expression of H+-ATPase were minor. By 21 days the remodelling process appeared to have returned to control levels. Conclusion: The findings of this investigation indicate that orthodontic relapse in the rat model occurs rapidly and remodelling of the alveolar bone plays a central role in the relapse processes of both actively moved, and adjacent teeth.

  Objectives: To investigate the mRNA levels of bone resorption and formation markers during orthodontic relapse of rat molars. Methods: Thirty-five maxillary right first rat molars were moved mesially (pressure side) by closed coil springs (0.5N force) whilst the contralateral maxillae served as internal controls. An additional non-treated control group (n=5) was used. Following ten days of orthodontic tooth movement (OTM) all appliances were removed, the amount of OTM registered and five animals were sacrificed. The molars of the remaining animals were allowed to relapse for one, three, five, seven, 14 and 21days. Orthodontic relapse was then measured and the animals sacrificed. RNA was isolated from bone removed from the mesial aspect of all maxillary first molars using a biopsy punch. Real-time PCR was performed for three housekeeping genes: 18S rRNA, glyceraldehyde-3-phosphate dehydrogenase and β-actin, and seven target genes: runx2, collagen type I (coll-I), alkaline phoshatase (ALP), osteocalcin, vacuolar type proton ATPase (H+-ATPase), tartrate-resistant acid phosphatase (TRAP) and receptor activator of nuclear factor kappa β ligand (RANKL). Results: One day after appliance removal, the molars relapsed to a mean 73% of the achieved OTM at ten days of active treatment, and then steadily relapsed to a mean 93% at 21 days. Levels of RANKL, TRAP and H+-ATPase were increased on the experimental side from the end of active OTM to 7 days relapse indicating bone resorption. They approached contralateral side levels at 14 and 21 days. Levels of bone formation markers coll-I, ALP, runx2 and osteocalcin showed little differences between the experimental and contralateral sides up to 14 days and then began to increase on the experimental side. Conclusions: On the former pressure side bone resorption continues during the early periods of orthodontic relapse, whilst bone formation does not occur until two weeks after the start of orthodontic relapse.
