Background
Temporomandibular joint (TMJ) discectomy and disc repositioning surgery (DRS) remain treatment options for disc displacement (DD) when conservative and minimally invasive treatments fail. DRS and discectomy with disc replacement are used, in part, to prevent osteoarthritis (OA) development.
Objective
To assess evidence for OA development after surgical treatment of TMJ DD by comparing OA development in surgically treated TMJs to OA development in conservatively treated patients with DD.
Methods
A systematic search (Medline, Embase and Cochrane up to 3 October 2024) was performed, and PRISMA was followed. A lack of effect sizes precluded meta-analysis; a descriptive statistical comparison between groups was made (Student's t-test, χ2 test, significance level p < 0.05).
Results
Eight observational studies, including discectomy of 117 TMJs (DD with/without reduction; without replacement), 206 comparison TMJs (DDwoR; observation) and one DRS study, met the eligibility criteria. An additional case report met the eligibility criteria but was not included in data synthesis. Heterogeneity in study designs, diagnostic methods and criteria, and mostly high Risk of Bias according to GRADE was found. OA and OA-related findings increased in all discectomy and comparison studies at follow-up.
Conclusion
A consistent trend of increasing TMJ OA frequency was observed across the included studies in both operated and unoperated TMJs with DD. Most studies provided evidence of very low certainty. This systematic review could therefore not reliably evaluate whether TMJ discectomy is itself responsible for OA development. Further research is needed to determine the influence of surgical treatments of TMJ DD on OA.

Frailty is associated with poorer health-related quality of life (HRQoL) in older adults, but studies rarely assess general and oral HRQoL concurrently in acute care populations. Objectives: To examine the associations between frailty and general as well as oral HRQoL among older adults admitted to a municipal in-patient acute care (MipAC) unit. Design: Cross-sectional study. Setting: Oslo MipAC unit, Norway. Participants: A total of 382 patients aged 70 years or older admitted to the MipAC unit between January and September 2023. Measurements: Frailty was assessed using the Clinical Frailty Scale (CFS). General HRQoL was measured with the 15D instrument and a single-item global life satisfaction question. Oral HRQoL was assessed using the Oral Health Impact Profile-14 (OHIP 14). Associations between frailty and HRQoL were analyzed using linear, ordinal logistic, and binary logistic regression. Exploratory analyses of 15D and OHIP-14 dimensions were performed using non-parametric trend tests. Results: Increasing frailty was significantly associated with lower 15D scores (adjusted B -0.053, 95% CI -0.061 to -0.045), higher odds of lower life satisfaction (adjusted OR 1.19, 95% CI 1.02 to 1.39), and higher odds of reporting oral health impacts on the OHIP-14 (adjusted OR 1.25, 95% CI 1.05 to 1.48). Most 15D dimensions declined with increasing frailty, except distress. For OHIP-14, significant associations were observed for functional limitation, physical pain, and psychological disability. Conclusions: Frailty assessed with the CFS was consistently associated with reduced general and oral HRQoL in older adults admitted to a MipAC unit. These findings support the clinical relevance of simple frailty assessment during acute admission and suggest that it may help identify patients at risk of poorer quality of life.

Objective: To compare systemic profiles, oral findings, and documented soft tissue healing between cancer and osteoporosis patients with medication-related osteonecrosis of the jaw (MRONJ).
Material and methods: This retrospective single-center cohort included 105 cases treated in 2013–2019 (56 cancer; 49 osteoporosis). Healing at last follow-up was analyzed using adjusted logistic regression and Firth sensitivity analysis.
Results: Of 102 cases with known final status, 68 had documented healing. Osteoporosis cases were older, predominantly female, and had longer antiresorptive exposure than cancer cases (76.7 ± 11.1 vs. 70.0 ± 8.5 years; 102.5 ± 77.8 vs. 34.6 ± 28.5 months; both p < 0.001). Stage II predominated and Stage 0 was uncommon (3/105, 2.9%). Previous tooth extraction at the MRONJ lesion site was more frequent in osteoporosis (95.9% vs. 71.4%; p < 0.001). Surgical treatment was associated with higher healing odds (adjusted odds ratio [OR]: 11.12, 95% confidence interval [CI]: 4.12 to 33.35; p < 0.001), whereas cancer indication was associated with lower odds (adjusted OR: 0.23, 95% CI: 0.07 to 0.68; p = 0.011).
Conclusions: Systemic profiles differed; oral patterns were similar. Non-random treatment and missing healing dates limit interpretation. Previous tooth extraction was the most frequent antecedent event; its indication and timing were unverified.

Background Oral findings are common among older adults, but it is unclear whether they vary with the duration of antiresorptive therapy (ART) exposure or relate to the severity of osteoporosis in postmenopausal women. Methods In this cross-sectional cohort ( n = 129), postmenopausal women undergoing osteoporosis assessment/follow-up were stratified by ART exposure duration: no current ART control (G1, n = 50), short ART (G2, 3–5 years; n = 39), and long ART (G3, > 5 years; n = 40). The prespecified primary oral endpoint was any mucosal finding ( oral_mucosal_any ), defined as ≥ 1 prespecified mucosal lesion category at oral examination. The oral_mucosal_any endpoint was analysed by component to distinguish traumatic from non-traumatic mucosal findings. Secondary oral endpoints included tooth and molar counts, periodontal findings, and the number of mucosal lesion categories. Group differences were analysed using age- and BMI-adjusted regression (G1 reference). Secondary analyses compared oral findings across fracture-burden strata and hip BMD/T-score categories and evaluated whether oral variables improved discrimination for hip osteoporosis (total hip T-score ≤ − 2.5) beyond age, BMI, and ART group. Results Mucosal findings were common and non-monotonic across ART groups (G1: 52.0%, G2: 30.8%, G3: 52.5%). Among the 59 participants with any mucosal finding, traumatic changes were present in 43 (72.9%); in a mutually exclusive classification, 38 (64.4%) had traumatic-only findings. In age- and BMI-adjusted models, short ART was associated with lower odds of oral_mucosal_any versus controls (OR 0.38, 95% CI 0.16–0.93; p = 0.035), whereas long ART did not differ (OR 0.94, 95% CI 0.38–2.33; p = 0.894). Tooth and molar counts, periodontal burden, and non-traumatic mucosal findings showed no robust graded pattern with ART duration. Oral findings were broadly similar across fracture-burden strata and hip BMD categories. Adding total teeth and oral_mucosal_any increased apparent Area Under the Curve (AUC) for hip osteoporosis from 0.795 to 0.820, but optimism-corrected AUC changed only from 0.736 to 0.741 (corrected ΔAUC = 0.005). Conclusions In this cross-sectional cohort, common oral findings did not show a robust graded duration–response pattern across ART exposure groups and were not consistently associated with fracture burden or hip BMD strata. The broad mucosal endpoint was largely driven by common traumatic findings and should be interpreted as a clinically pragmatic but biologically heterogeneous endpoint. Oral variables added negligible optimism-corrected discrimination beyond age, BMI, and ART group.

Objectives
Health literacy (HL) is an important determinant of health behaviors, including diet. In military populations, optimal nutrition is critical for physical performance, readiness, and long-term health. In Norway, first-time military service provides a unique opportunity to influence young conscripts’ health habits. This study investigates the relationship between HL and dietary patterns, body mass index (BMI), and meal adherence among Norwegian conscripts, with an emphasis on potential sex differences.
Study Design
A cross-sectional study.
Methods
This cross-sectional study included Norwegian conscripts completing first-time military service. HL was measured using the validated HLS-Q12 instrument. Dietary behaviors, meal patterns, and BMI were assessed using self-reported questionnaire data. A digital questionnaire was sent to Norwegian conscripts completing their first-time military service in 2022 (n=9,991). A total of 2,225 conscripts consented to participate in the study. The survey assessed dietary habits across different military settings, self-reported diet quality, and HL using the HLS-Q12 tool. BMI was calculated from self-reported height and weight. Statistical analyses included chi-square tests and Pearson correlation to explore associations between HL, diet, and BMI.
Results
Significant associations were observed between HL and several dietary variables; however, most associations were weak in magnitude. Higher HL was associated with more regular meal patterns and better self-reported diet quality. Additionally, differences between sexes were observed, but effect sizes were generally small. More than half of conscripts had an adequate HL, with males scoring significantly higher than females. Conscripts with adequate HL were more likely to rate their diet as good, follow meal recommendations, and maintain a normal BMI. However, no significant association between HL and BMI was found in the total population; differences appeared in sex-stratified analyses, among female conscripts. Independently of HL, unhealthy snacking and high-sugar drink consumption were more common during field training, whereas conscripts with inadequate HL (iHL) reporting higher sugary snack intake at home. Adequate HL was weakly but significantly correlated with higher self-reported diet quality, more frequent adherence to regular meals, and lower sugary snack consumption. This could mean that HL was associated with self-reported dietary intentions but not outcomes like BMI.
Conclusions
HL may be associated with dietary behaviors among conscripts. However, given the weak associations and study limitations, the findings should be interpreted with caution. Although more than half of the conscripts had adequate HL, iHL was prevalent among Norwegian conscripts. Poorer self-reported diet quality and lower adherence to regular meal patterns is associated with iHL, particularly during field training. No significant associations was found between HL and BMI. The findings suggest a need for tailored education to improve HL and behavior change, to promote healthier dietary habits during military service.

Purpose: Dry eye disease (DED) is a multifactorial condition characterized by the loss of ocular surface homeostasis, causing discomfort and ocular symptoms. Higher age, female sex, and meibomian gland dysfunction (MGD) are some of the reported risk factors for DED. However, age-specific studies of DED are rare. This study aimed to determine the prevalence of DED and MGD in a cohort of Norwegians in their mid-60s. Additionally, to investigate potential sex-related differences in the prevalence of DED and MGD.
Methods: For this cross-sectional study, a random sample of 137 demographically homogeneous Norwegian residents of Oslo in their mid-60s was recruited. Participants completed the Ocular Surface Disease Index (OSDI) dry eye symptom questionnaire and underwent clinical dry eye examinations. DED was defined as OSDI ≥ 13, accompanied by fluorescein break-up time (FBUT) < 10s and/or ocular surface staining (OSS) ≥ 1. The presence of MGD was defined as a score of 1 for both meibum quality (MQ) and meibum expressibility (ME) or a score > 1 for either MQ or ME.
Results: The overall prevalence for DED was 20.4% [95% CI: 14.0 to 28.2], and for MGD was 90.5% [95% CI: 84.3– 94.9]. No significant sex differences were observed for either condition. MGD was highly prevalent among both DED and non-DED participants: all individuals with DED (n = 28) had MGD, while among those without DED (n = 109), prevalence was 88.1% (males: 85.7%; females: 90.0%). No significant correlation was found between MGD and sex (rs ([137] = [0.075], p = 0.413). In the whole cohort, OPI and FBUT correlated positively with ME (rs ([132] = [0.437] and rs ([132] = [0.296], both p < 0.001) and negatively with OSS (rs ([132] = [− 0.315] and rs ([132] = [0.423], both p< 0.001) and lid margin abnormality (rs ([130] = [− 0.222], p = 0.011; rs ([132] = [0.234], p = 0.007). FBUT correlated also positively with tear production (rs ([128] = [0.308], p< 0.001). A more frequent use of topical ocular lubricant was found among females (p = 0.043). In the symptomatic group, females had significantly lower OPI and FBUT values (p = 0.046 and 0.027, respectively). Among symptomatic participants, categorized as mild (OSDI 13– 22), moderate (OSDI 23– 32), and severe (OSDI 33– 100), only in the severe group did females show higher OSDI scores (p= 0.042) but lower OSS (p= 0.031) compared to males.
Conclusion: In this cohort of Norwegians in their mid-60s, DED and MGD were highly and equally prevalent among both sexes. The high prevalence of these conditions underscores the need to raise awareness of DED and MGD within the young elderly population.

Background: Dry mouth is a common and burdensome condition in older adults and its severity may increase with use of anticholinergic medications. The aims were to describe anticholinergic medication use among acutely ill older adults, examine the association between anticholinergic burden and dry mouth severity using the composite Dry Mouth Severity Points (DMSP) measure and investigate whether the association differs by sex.
Methods: Patients ≥70 years admitted to Oslo municipal inpatient acute care unit were eligible. Anticholinergic burden (ACB) was assessed using the ACB calculator and classified as 0, lower (ACB = 1-2) or high (ACB = ≥ 3). Dry mouth severity was quantified with DMSP, range 0-4, assigning one point for each indicator above cut-off: General Xerostomia Question ≥ 3, Summated Xerostomia Inventory ≥ 11, Clinical Oral Dryness Score ≥ 6 and unstimulated whole saliva secretion rate ≤ 0.1 mL/min. Associations were examined with ordinal logistic regression.
Results: Of 382 examined patients, 256 (mean age 84 ± 7 years, 70% women) had complete data. Mean [standard deviation (SD)] ACB score was 2.0 (1.7); 18% had ACB = 0, 50% had ACB = 1-2 and 32% had ACB = ≥ 3. DMSP scores of 0, 1, 2 and ≥ 3 were present in 29%, 36%, 23% and 12%, respectively. Compared with patients with ACB = 0, those with ACB = 1-2 [odds ratio (OR) = 2.06, 95% confidence interval (CI) 1.07-3.97) and ACB = ≥ 3 (OR = 2.25, 95% CI 1.09-4.63) had higher odds of more severe dry mouth. Women had significantly higher ACB and DMSP scores than men.
Conclusions: Even lower anticholinergic burden was associated with greater dry mouth severity, highlighting the need for early recognition and proactive management in acutely ill older adults.

Background
This study investigates unmet supportive care needs (SCNs) among head and neck cancer (HNC) survivors beyond 5 years after diagnosis and examines the association with sociodemographic, clinical and lifestyle factors, and differences in European and non-European regions and healthcare systems.
Methods
In this cross-sectional study, 1097 HNC survivors from 11 countries completed the Short-Form Supportive Care Needs Survey (SCNS-SF34) and HNC-specific module (SCNS-HNC), encompassing physical and daily living, psychological, sexuality, HNC-specific and lifestyle domains. Scores were dichotomized per domain and across domains into moderate-high unmet SCNs (yes/no). Logistic regression analyses were used to investigate associated factors.
Findings
Half (50%, proportion 100/200) of HNC survivors had unmet SCNs (overall), especially unmet HNC-specific (40%, 40/100), psychological (25%, 25/100), and physical and daily living (22%, 22/100) needs. Personal (women, lower age), lifestyle (smoking, alcohol consumption) and clinical factors (advanced tumor stage, second primary tumor, multimodality treatment [versus single surgery], poor Karnofsky performance score and comorbidities) were associated with unmet SCNs. Physical and daily living, HNC-specific and overall unmet SCNs were more likely among survivors from Northern Europe compared to Southern and Western Europe. Unmet psychological, sexuality and lifestyle needs were more likely among non-European countries. All unmet SCNs (except psychological) were more likely among survivors with a national health system compared to a social and/or etatist health insurance system.
Interpretation
Half of HNC survivors have unmet SCNs. Insight into healthcare utilization may provide insights how to improve care.

Purpose Our aim was to investigate oral health-related quality of life (OHRQoL) and toxicities in long-term head and neck cancer (HNC) survivors diagnosed ≥five years earlier. Methods HNC survivors treated between 2007 and 2013 participated in an international cross-sectional study. They completed the European Organization for Research and Treatment of Cancer (EORTC) core quality of life questionnaire (EORTC QLQ-C30) and the oral health module (EORTC QLQ-OH15) and attended a hospital examination. Clinicians scored toxicities using the Common Terminology Criteria for Adverse Events version 5.0. OHRQoL was analyzed based on four types of treatment: surgery, radiotherapy, chemoradiotherapy without surgery and surgery with postoperative (chemo) radiotherapy. Survivors were divided into three groups according to the EORTC QLQ-OH15 oral health-QoL scale score; the lowest (defined as poor OHRQoL), middle and highest tertile. Results Eleven sites in six countries enrolled 404 HNC survivors. The median time since diagnosis was 8.4 years, the mean age was 66 years and 67% were male. A total of 116 (29%) of the survivors reported poor OHRQoL. They were more often females and survivors with advanced disease. The survivors with poor OHRQoL also had more toxicity: dysphagia, trismus, osteonecrosis of the jaw, oral pain and dry mouth. There were no clinically significant differences in OHRQoL between the four treatment groups. Conclusion Our study showed that the survivors who reported poor OHRQoL also had a high level of late toxicity. This highlight the need for improved follow-up of the oral health of HNC survivors many years after initial treatment, especially for women and those who were treated for advanced disease.

Background & aims
While the management of olfactory disorders among patients with post Coronavirus Disease (COVID-19) is well elucidated, treatment of taste dysfunction, oral burning, and oral dryness is less extensively studied among post-COVID-19 patients. This prospective study aimed (i) to assess the prevalence of chemosensory, trigeminal, and salivary dysfunctions in patients at two different time points following COVID-19 infection, and (ii) to evaluate the recovery process over time.
Methods
In this cross-sectional study, 54 Early-post-COVID-19 (3–11 months since COVID-19), 46 Late-post-COVID-19 (≥12 months since COVID-19) and 76 Non-COVID-19 controls were recruited. Participants’ olfactory, gustatory, trigeminal, and salivary status were investigated. Depending on the test outcomes, tailored treatments were administered, and patients were monitored every 3–6 months.
Results
The findings indicated that none of these dysfunctions improved on their own over time. Patients in both post-COVID-19 groups demonstrated significantly reduced smell, trigeminal and salivary functions compared to the control group. Taste impairment was significantly more severe in Late-post-COVID-19 patients compared to Early-post-COVID-19 patients. Follow-up evaluations revealed significant improvement in both smell and taste functions at each follow-up visit across both patient groups.
Conclusion
Chemosensory, trigeminal, and salivary disorders in post-COVID-19 patients may not resolve with time alone, underscoring the need for tailored treatments to achieve significant improvements.

Objectives
This study evaluates the safety and osteoconductive potential of a ceramic TiO2 scaffold in human alveolar bone, hypothesising that placement in human extraction sockets is safe, supports bone ingrowth and preserves ridge dimensions.
Material and Methods
Nine subjects requiring the extraction of ≥ 1 premolar or molar tooth were enrolled. Full-thickness mucoperiosteal flaps were elevated. Following tooth extraction, porous ceramic TiO2 scaffolds were adjusted chair-side, placed into the extraction socket and covered by a resorbable collagen membrane. After 6 months of healing, before dental implant placement, a biopsy was performed at the site where the scaffold had been placed. Histomorphometric and micro-CT analyses were made. CBCT images were made at baseline and at 6 months to assess alveolar ridge dimensional changes. Implants placed in regenerated bone were followed for 21 months.
Results
Scaffold customisation with chairside adjustment was straightforward. No serious adverse events were observed during the 6 months following scaffold implantation. One scaffold was removed due to an early exposure, while the remaining 9 scaffolds successfully integrated. Two sites presented with minor soft tissue dehiscences at 1–2 weeks, which healed spontaneously. Micro-CT and histology confirmed new bone formation. Soft tissue healing was favourable, with no adverse reactions. Dimensional analysis revealed minimal changes in alveolar ridge width and height, indicating effective preservation of volume.
Conclusions
This clinical study confirms that TiO2 scaffolds are a safe and effective option for promoting bone regrowth and preserving volume in human alveolar bone, thereby facilitating bone formation and maintaining ridge dimensions post-extraction.

Ambulance clinicians may have to apply coercion while delivering healthcare. However, coercion is a source of stress for
all involved. Challenges include unclear legislation, lack of training, equipment, support, not least ethical dilemmas. This
paper explores factors influencing ambulance clinicians’ experiences of exercising coercion. An anonymous online survey
was conducted between Oct 2021 and Nov 2022. During a six months period, 488 ambulance clinicians reported using
coercion, and 431 answered a free-text response question about their experience. Using Graneheim & Lundman's analysis of qualitative content, 376 answers were analyzed. Three overarching themes were identified. First, ambulance clinicians reflected on what constitutes healthcare and how they perceive their professional role, including the idea that
coercion can, in some cases, be a legitimate form of healthcare; at the same time, clinicians expressed concern about
patients’ experiences, and described feelings of powerlessness. Second, participants highlighted encounters and situations
that they found especially distressing, often due to ethical ambiguity. Third, the experience of exercising coercion was
shaped by various contextual and structural conditions concerning equipment, legislation, insufficient training and expertise, as well as the complex impact of police involvement during critical incidents and lack of support. Social identity theory
was used to understand how the described challenges influenced the use of coercion and shaped ambulance clinicians’
professional identity at the individual, service, and societal levels. This identity was challenged across all levels.
Clinicians often found themselves in situations where they felt morally conflicted, underprepared, and isolated from
the wider healthcare system. These findings underline the need for integrated strategies that address coercion through
a multi-level lens. Clinical implications include the implementation of structured ethical reflection, improved training,
adequate equipment, and clear, context-specific guidelines. Such efforts can strengthen ethical competence, reduce
moral distress, and align prehospital practice with broader health-led efforts to minimize coercion.

Background
Oral health is an essential part of overall health and well-being in older adults and may be influenced by medications and frailty. This study aimed to describe the prevalence of xerostomia and hyposalivation and examine their associations with xerogenic medications and frailty in acutely ill older adults.
Methods
This cross-sectional study included patients aged ≥ 70 years admitted to the Municipal In-Patient Acute Care unit in Oslo, Norway. General xerostomia was defined as a score ≥ 3 on the General Xerostomia Question (GXQ). Specific xerostomia was measured with the Summated Xerostomia Inventory (SXI). Objective dry mouth measures included unstimulated whole saliva (UWS) secretion rate and the Clinical Oral Dryness Score (CODS), with hyposalivation defined as UWS ≤ 0.1 mL/min. Examination of dentition included numbers of missing teeth and posterior occluding pairs of teeth. Medication data included total drug burden and number of xerogenic medications. Frailty was assessed using the Clinical Frailty Scale (CFS). Associations were analyzed using logistic regression models.
Results
Among 382 patients (mean age 84 years, 72% women), general xerostomia was present in 30% and hyposalivation in 56%. Mean number (SD) of xerogenic medications was 3.7 (±2.2). The number of xerogenic medications used was significantly associated with general xerostomia in both unadjusted (OR: 1.17, 95% CI: 1.05–1.29) and adjusted models (OR: 1.13, 95% CI: 1.01–1.26). Median UWS declined numerically with increasing number of xerogenic medications. Higher CFS scores were associated with increased subjective xerostomia scores (GXQ and SXI), elevated CODS, more missing teeth, and reduced occlusion, but not with general xerostomia or hyposalivation when predefined cut-off values were applied.
Conclusion
Xerostomia and hyposalivation were common among acutely ill older adults. General xerostomia was associated with xerogenic medication use and frailty. Our findings highlight the need to incorporate oral health assessments into routine geriatric care and emphasize the importance of interdisciplinary collaborations.

Objectives: Sjögren’s disease (SjD) and systemic lupus erythematosus (SLE) share genetic risk at the DDX6-CXCR5 locus (11q23.3). Identifying and functionally characterising shared SNPs spanning this locus can provide new insights into common genetic mechanisms of autoimmunity.
Methods: Transdisease meta-analyses, fine-mapping, and bioinformatic analyses prioritised shared likely functional single nucleotide polymorphisms (SNPs) for allele-specific and cell type–specific functional interrogation using electromobility shift, luciferase reporter, and quantitative chromatin conformation capture assays and clustered regularly interspaced short palindromic repeat (CRISPR) gene regulation.
Results: Five shared SNPs were identified as likely functional in primary human immune cells, salivary gland and kidney tissues: rs57494551, rs4936443, rs4938572, rs7117261, and rs4938573. All 5 SNPs exhibited cell type-specific and allele-specific effects on nuclear protein binding affinity and enhancer/promoter regulatory activity in immune, salivary gland epithelial, and kidney epithelial cell models. Mapping of chromatin–chromatin interactions revealed a chromatin regulatory network that expanded beyond DDX6 and CXCR5 to include PHLDB1, lnc-PHLDB1-1, BCL9L, TRAPPC4, among others. Coalescence of functional assays and multiomic data analyses indicated that these SNPs likely modulate the activity of 3 regulatory regions: intronic rs57494551 regulatory region, intergenic SNP haplotype (rs4938572, rs4936443, and rs7117261) regulatory region, and rs4938573 regulatory region upstream of the CXCR5 promoter.
Conclusions: Shared genetic susceptibly at the DDX6-CXCR5 locus in SjD and SLE likely alters common mechanisms of autoimmunity, including interferon signalling (DDX6), autophagy (TRAPPC4), and lymphocytic infiltration of disease-target tissues (CXCR5). Further, using multiomic data from patients with SjD, combined with bioinformatic and in vitro functional studies, can provide mechanistic insights into how genetic risk influences the biological pathways that drive complex autoimmunity.

Objectives Measurement of tear film stability is central in dry eye disease (DED) diagnosis. In this study, we aimed to compare the performance of two methods of tear film stability measurement: non-invasive tear break-up time (NIBUT) and fluorescein tear film break-up time (FTBUT).
Design Cross-sectional study.
Setting and participants The study involved 132 subjects of 65-year-old inhabitants of the Oslo region who were not seeking ophthalmic care.
Interventions The participants underwent a battery of DED tests, including NIBUT measured on Oculus Keratograph 5M and a traditional method using fluorescein drops (FTBUT). Oculus Keratograph 5M measures two types of NIBUT:; appearance time of the first dry spot (NIBUTFirst) and average NIBUTAvg.
Results 74 participants (56%) were female and 58 were male (44%). Subjects presented with varying degrees of DED signs and symptoms. Mean values of NIBUTFirst and FTBUT from all the participants were significantly different (6.2±4.9 s vs 8.6±6.2 s, p<0.0001). There was also a significant difference between NIBUTFirst and NIBUTAvg values (6.2±4.9 s vs 8.3±5.5 s, p<0.0001). In contrast, no difference was observed between FTBUT and NIBUTAvg values (8.6±6.2 s vs 8.3±5.5 s, p=0.655). The receiver operating characteristic curve analysis was performed to compare NIBUT and FTBUT in regards to other clinical tests (Ocular Surface Disease Index, ocular surface staining, blink interval, eye redness, corneal sensitivity, lid debris, Schirmer I test, tear osmolarity, meibum quality, meibum expressibility, lid hyperemia, tear meniscus height,. irregular lid margin, conjunctival hyperaemia, margin telangiectasia, lipid layer and meibomian gland drop-out). While FTBUT demonstrated results with area under the curve>0.6, neither NIBUTFirst nor NIBUTAvg showed significant results.
Conclusion NIBUTFirst was shorter than FTBUT. Low correlation between NIBUT and FTBUT indicates that these diagnostic tests are not interchangeable. Other DED tests had correlation, though low, while NIBUT did not demonstrate correlation.

Head and neck cancer (HNC) patients may experience toxicities as a result of their treatment modality. While acute toxicities have been well documented, the prevalence of toxicities at long-term follow-up of HNC survivors is less clear. As part of a multi-national, cross-sectional study, HNC survivors at least 5 years post-diagnosis were invited to undergo a toxicity examination. Using the Common Terminology Criteria for Adverse Events (version 5), 33 toxicities were assessed. From 2019 to 2021, 1094 survivors from 26 sites in 11 countries completed the assessment. Eighty-seven percent were from Europe, and most were survivors of oropharynx (35%), oral cavity (21%), or larynx cancer (19%). The majority had been diagnosed at stage III or IV (62%), and the median time since diagnosis was 8 years (range 5–36). Most had been treated with surgery and radiotherapy with or without chemotherapy (38%). Six percent had no toxicities, and 26% had only mild toxicities. 68% had at least one moderate or severe late toxicity. Overall, the most frequent late toxicities at any grade were dry mouth (67%), soft tissue fibrosis (52%), dysphagia (51%), and voice alterations (39%). Fistulae, neck and face edema, and osteonecrosis of the jaws were present in very few survivors. Our study shows that the majority of HNC survivors experience moderate or severe late toxicities, but that the problems are concentrated in a small group of specific toxicities. Understanding the problems experienced in the long term can help better inform newly diagnosed patients as well as inform survivorship follow-up initiatives.

Primary Sjögren’s syndrome is a chronic inflammatory disease characterized by the destruction of exocrine glands. We have previously shown significantly upregulated levels of CXCL10 and CCL3 chemokines in saliva from Sjögren’s syndrome patients. In this study, we examined the expression pattern and localization of these chemokines at the site of inflammation in patients’ minor salivary glands using novel RNAscope® in situ hybridization. Minor salivary glands from 33 primary Sjögren’s syndrome patients and 22 non-Sjögren’s syndrome (non-SS) sicca controls were included. The biopsies were formalin-fixed, paraffin-embedded, and histopathologically evaluated. The CXCL10 and CCL3 mRNA expression in the glandular tissue was investigated using reverse transcription quantitative real-time polymerase chain reaction followed by an RNAscope® in situ hybridization. The mRNA expression of CXCL10 was higher than CCL3 in all patients. Significantly elevated expression of CXCL10 and CCL3 was detected in patients that also expressed autoantibody positivity and a positive biopsy for mononuclear cell infiltrates when compared with non-SS sicca controls. CXCL10 was localized as clusters within focal infiltrates as well as adjacent to acinar and ductal epithelium, while CCL3 was expressed as scattered single mRNA molecules in focal infiltrates and in acinar cells. Our findings suggest CXCL10 as a possible disease biomarker in primary Sjögren’s syndrome due to its upregulated expression in both saliva and minor salivary glands of patients and the localization in the tissue. This should be re-assessed in a larger primary Sjögren’s syndrome patient cohort, followed by additional functional studies to further validate its potential as a disease biomarker.

Introduction Minimal important change estimates (MIC) are useful for interpreting results of clinical research with quality of life (QoL) as an endpoint. For the European Organisation for Research and Treatment of Cancer head and neck cancer module, the EORTC QLQ-HN43, no such thresholds are established. Methods Head and neck cancer patients under active treatment (n = 503) from 15 countries completed the EORTC QLQ-HN43 three times (t1: before treatment, t2: three months after t1, t3: six months after t1). A subgroup completed a Subjective Significance Questionnaire (SSQ), indicating experienced change from the previous time point in four QoL domains. QoL was assumed to deteriorate after t1 and improve again until t3. The MIC was established using the average of mean differences in SSQ groups (MICmean) and estimates based on logistic regressions (MICpredict). Additionally, minimal detectable changes (MDC) were computed using 0.5 standard deviation and standard error of the mean. Results For swallowing, speech, dry mouth, and global QoL, the MIC for deterioration were 13, 14, 26, and 10 respectively. The MIC for improvement were 8 (swallowing), 6 (dry mouth), and 5 (global QoL); no MIC for speech improvement can be presented because of insufficient correlation between change score and anchor. The MDC estimates for deterioration were 15, 14, 15, and 11. For improvement, the MDC estimates were 13, 14, 14, and 11. Conclusions Our results underline that no single MIC or MDC can be applied to all EORTC QLQ-HN43 scales, and that the MIC for deterioration seems larger than those for improvement.

Between the years 2008 and 2023, 306 patients were referred to the Department of Oral Surgery and Oral Medicine, Faculty of Dentistry, University of Oslo, for salivary gland biopsy after clinical suspicion of Sjögren’s syndrome. This study aimed to investigate possible associations between serology and minor salivary gland histopathology by stratifying patients according to serology features (negative, single, or double seropositivity according to anti-SSA and anti-SSB), focus score (FS, negative, or actual score), and germinal centers (GCs). Information was collected from referrals and visits. Collected information included oral and ocular dryness symptoms, sialometry, Schirmer I test, histopathological analysis, and serological findings. Patients were classified using the 2002 American European Consensus Group classification criteria and were stratified into seven subgroups according to their serological profiles and FS, which were compared. The majority of patients were females, and the most frequently referred age group was 50–59 years of age. Most patients had both oral and ocular symptoms. Seropositive patients had higher median FS compared to seronegative patients. Double-seropositive patients had significantly higher median FS compared to single-seropositive patients. The results indicated that anti-SSB together with anti-SSA plays a role in disease severity in the salivary glands. In addition to anti-SSA, anti-SSB should be routinely evaluated as a possible predictor of the severity of inflammatory destruction in the salivary gland tissue.

Background There is lack of evidence on chronic fatigue (CF) following radiotherapy (RT) in survivors of head and neck cancer (HNC). We aimed to compare CF in HNC survivors > 5 years post-RT with a reference population and investigate factors associated with CF and the possible impact of CF on health-related quality of life (HRQoL). Material and methods In this cross-sectional study we included HNC survivors treated in 2007–2013. Participants filled in patient-reported outcome measures and attended a one-day examination. CF was measured with the Fatigue Questionnaire and compared with a matched reference population using t-tests and Cohen’s effect size. Associations between CF, clinical and RT-related factors were investigated using logistic regression. HRQoL was measured with the EORTC Quality of Life core questionnaire. Results The median age of the 227 HNC survivors was 65 years and median time to follow-up was 8.5 years post-RT. CF was twice more prevalent in HNC survivors compared to a reference population. In multivariable analyses, female sex (OR 3.39, 95 % CI 1.82–6.31), comorbidity (OR 2.17, 95 % CI 1.20–3.94) and treatment with intensity-modulated RT (OR 2.13, 95 % CI 1.16–3.91) were associated with CF, while RT dose parameters were not. Survivors with CF compared to those without, had significantly worse HRQoL. Conclusions CF in HNC survivors is particularly important for female patients, while specific factors associated with RT appear not to play a role. The high CF prevalence in long-term HNC survivors associated with impaired HRQoL is important information beneficial for clinicians and patients to improve patient follow-up.

Introduction The acquisition of skills and manual dexterity in aesthetic dentistry in undergraduate teaching requires preclinical practices with simulation that should approximate real clinical situations. Objective We will present a digital procedure for the creation of three-dimensional (3D) resin models adapted to the practices of the Aesthetic Dentistry course. Material and method Stereolithography or Standard Tesellation Language (STL) files of a real patient were previously obtained with a 3-shape intraoral scanner. Using the Exocad computer programme for dental prosthesis design, various modifications were made, such as incisor rotation, surface alterations imitating dysplasias or erosions, the creation of diastemas, or even changes in tooth size. The virtual model was printed in resin for use by students. Once the practices were finished, the students and the teachers evaluated the use of the 3D printed models. Results The result is the typodont model, in which seven laboratory sessions took place: 1-Restoration of conoid tooth morphology, 2-Cervical abrasion/ erosion restoration, 3-Direct Composite Veneer, 4-Aesthetic correction in a tooth with rotation, 5-Diastem closure, 6-Occlusal abrasions/ erosion, and 7-Maryland bridge. 90.48% of the students evaluated the designed 3D model as the best method for laboratory practice compared to other methods, obtaining a general assessment of 8.3 out of 10. Conclusion The method used has provided a reproducible standard analog model for direct aesthetic dental restoration practice, with a good assessment by students and teachers.

Objectives To calculate a polygenic risk score (PRS) based on single nucleotide variants (SNVs) previously associated with primary Sjögren’s disease (SjD) with genome-wide significance and determine the genetic risk for SjD stratified by antibodies, sex and age at diagnosis. Methods Patients with SjD (n = 1065) were genotyped using Illumina OmniExpressExome chip. Control genotype data were available (n = 7742). Two PRSs were constructed, one including HLA gene variants (n = 21 SNVs), and one without HLA (n = 18 SNVs). High PRS quartile (Q4) individuals were compared with low PRS (Q1–3). Results A high PRS was associated with SSA antibody-positive SjD (OR 9.16, 95% CI 7.75–10.85, P = 3.7 × 10−146), and strengthened in SjD positive for both SSA/SSB antibodies (OR 13.67, 95% CI 10.88–17.32, P = 4.6 × 10−108). High PRS classified SSA/SSB antibody-positive SjD with very good accuracy (AUC 0.86). PRS without HLA showed a weaker association with SSA/SSB positive SjD (OR 2.09, 95% CI 1.71–2.55, P = 6.4 × 10−13). Antibody negative SjD displayed a PRS similar to controls. Patients in the high PRS quartile were significantly younger at diagnosis, 48.9 ± 14.9 vs 53.4 ± 13.4 years in the low PRS quartiles (Q1–3), P = 2.2 × 10−6, and presented higher frequencies of ANA, SSA and SSA/SSB antibodies, P < 1 × 10−5. Conclusion A high PRS is associated with SSA/SSB antibody positivity and early disease onset, both largely attributed to the weight of the HLA alleles. Integration of PRS with other biomarkers applied to clinical phenotypes could be a useful tool for disease risk stratification and treatment decisions.

Background Symptom assessment is key to effective symptom management and palliative care for patients with advanced cancer. Symptom prevalence and severity estimates vary widely, possibly dependent on the assessment tool used. Are symptoms specifically asked about or must the patients add them as additional symptoms? This study compared the prevalence and severity of patient-reported symptoms in two different versions of a multi-symptom assessment tool. In one version, three symptoms dry mouth, constipation, sleep problems were among those systematically assessed, while in the other, these symptoms had to be added as an “Other problem”. Methods This retrospective cross-sectional study included adult patients with advanced cancer at an inpatient palliative care unit. Data were collected from two versions of the Edmonton Symptom Assessment System (ESAS): modified (ESAS-m) listed 11 symptoms and revised (ESAS-r) listed 9 and allowed patients to add one “Other problem”. Seven similar symptoms were listed in both versions. Results In 2013, 184 patients completed ESAS-m, and in 2017, 156 completed ESAS-r. Prevalence and severity of symptoms listed in both versions did not differ. In ESAS-m, 83% reported dry mouth, 73% constipation, and 71% sleep problems, but on ESAS-r, these symptoms were reported by only 3%, 15% and < 1%, respectively. Although ESAS-r severity scores for these three symptoms were higher than on ESAS-m, differences did not reach statistical significance. Conclusion We identified significant differences in patient symptom reporting based on whether symptoms like dry mouth, obstipation and sleep problems were specifically assessed or had to be added by patients as an “Other problem”.

Background More than 70% of respondents in a previous survey among paramedics reported use of coercion or physical force towards patients. Coercion outside hospital is not permitted, and neither routines nor equipment intended for physical restraint is available in the Norwegian ambulance services. Paramedics carry out assignments involving use of force and coercion on unclear legal grounds, with no training in techniques or proper equipment. Attitudes and experiences of healthcare workers regarding incidents involving coercion in mental health care services are frequently reported in the research literature, yet little is known about para- medics’ experiences, and which factors contribute to their moral stress. Methods In the period June-August 2021, almost 400 employees in the ambulance services in a county in the eastern part of Norway were invited to answer a digital questionnaire. One question had an open text field with the question "Can you say something about how you experience transporting patients where force has to be used to secure the patient during transport?". The answers were analyzed using Graneheim and Lundman’s content analysis. Results We received eighty-five completed responses (response rate 21%). Force was used by 62 paramedics. Twenty-three left the text field open. The answers showed many unique responses. Content analysis resulted in two overarching themes; 1) lack of routines, equip- ment, and training regarding use of coercion and force in the ambulance service, and 2) paramedics were confronted with ethical dilemmas, alone and without support from legisla- tion or management. Conclusions The paramedics experienced discomfort related to the exercise of force and coercion during ambulance assignments due to the experience of unclear legislation, lack of training, rou- tines, and equipment in addition to frequent ethical dilemmas and the concern about lack of support from the employer. A clearer legal basis, adapted equipment in the ambulance and regular training, will contribute to greater security in the performance of the work, which will provide safer and more caring treatment for the patients and less moral stress for the staff. With established routines, the employer will be implicitly obliged, and paramedics will be safer in the performance of their work. Ethical reflection must be offered and put into a system.

Methods Twenty-two discectomy patients at the University of Oslo, Norway, with records confirming initial TMJ diagnosis and attendance of 10-year radiological follow-up were evaluated and eligible for 30-year follow-up. Primary variables: discectomy and CT-/CBCT-diagnosed OA at follow-ups. Secondary variables: perioperative TMJ diagnoses and remodelling at follow-up. Unoperated TMJs (Unop-TMJs) in unilaterally operated patients were controls. Statistical association and correlation analyses were performed for the 10-year follow-up (significance level p < 0.05). Results Twenty-two patients attended the 10-year follow-up (mean follow-up 11 years) with 27 operated TMJs (Op-TMJs) and 17 Unop-TMJs. OA perioperatively was associated with DD without reduction (p = 0.001) and additional disc abnormalities (p = 0.016). Although statistically non-significant, the number of TMJs with OA had increased at 10-year follow-up (p = 0.114, Op-TMJs: 14 to 20 joints; Unop-TMJs: 2 to 5 joints). Remodelling was correlated with discectomy (p = 0.003) and to OA (p = 0.006). Nine patients attended the 30-year follow-up (mean follow-up 32 years, 11 Op-TMJs). All TMJs with OA at 30-year follow-up had OA at 10-year follow-up. Mean maximal interincisal opening was 39 mm. No DC-TMD-diagnosed arthralgia was found. Conclusion Osteoarthritis developed similarly between Op- and Unop-TMJs. Only remodelling, not OA, was correlated to the surgery. The clinical results were still favourable at final follow-up.

Introduction - Although health information is widely available to the Norwegian population a lack of health literacy is still common and considered a public health challenge. However, there is limited information on the health literacy of the adolescent population in Norway. The purpose of this pilot study was to assess health literacy and self-reported health habits among soldiers in basic military training, both in field, camp, and home settings. Furthermore, we aimed to investigate which health habits characterize soldiers with inadequate health literacy.<p> <p>Methods - An electronic questionnaire was sent out to conscripts who completed first-time military service in 2022 and was answered by 2,225 conscripts. Descriptive statistics were performed using cross-tabulations, correlations, and chi-squared tests. The survey is part of an internal quality study of the Norwegian Armed Forces routines. It is approved by the Norwegian Armed Forces joint medical services.<p> <p>Results - In the study population, 43% scored inadequate health literacy on the European Health Literacy Survey Questionnaire-Q12 (HLS-Q12), and 57% scored adequate health literacy. We found a significant difference between those having inadequate and adequate HLS-Q12 scores in categories of self-rated health habits of oral health and diet with lower scores in the inadequate score group. The use of tobacco during field exercise in camp and at home was higher in the group of conscripts with adequate health literacy scores. The conscripts generally reported a lower standard of health habits during field exercises with significantly less brushing and flossing, higher use of tobacco products and higher consumption of energy drinks and sugary snacks compared to reported habits in camp and at home.<p> <p>Conclusion - The results from our study suggest that the conscripts in the Norwegian Armed Forces have a lower health literacy than the general population, and this is reflected in reported higher frequency of unhealthy habits in service settings than at home. The Armed Forces have an opportunity to take responsibility to optimize the health skills of their soldiers and to encourage them to make informed decisions about their own health. Improved adaptation to the national recommendations regarding nutrition, tobacco use, and oral health are areas having the potential to greatly improve the overall health habits of conscripts.

Background Surgical treatment of temporomandibular joint (TMJ) disc displacement (DD) has been established in different forms since over a century. Ther is a consensus to perform minimal invasive interventions as first-line surgical treatment since there are no evidence on best surgical practice yet. Objective The aim was to perform a complex systematic review (SR) on the topic—is there evidence for surgical treatment of TMJ DD? Methods The PICO was defined as DD patients (population), treated with different surgical interventions including arthrocentesis (intervention), compared with other or no treatment (control) regarding the outcome variables mandibular function, mouth opening capacity, TMJ pain, etcetera (outcome). For identification of prospective controlled trials and SRs, a search strategy was developed for application in three databases. Results The search yielded 4931 studies of which 56 fulfilled the stipulated PICO. Studies with low or moderate risk of bias were possible to include in meta-analyses. There were evidence suggesting arthrocentesis being more effective compared to conservative management (maximum interincisal opening (MIO): p < .0001, I2 = 22%; TMJ pain: p = .0003, I2 = 84%) and arthrocentesis being slightly more effective than arthrocentesis with an adjunctive hyaluronic acid injection (MIO: p = .04, I2 = 0%; TMJ pain: p = .28, I2 = 0%). Other treatment comparisons showed nonsignificant differences. The performed meta-analyses only included 2–4 studies each, which might indicate a low grade of evidence. Conclusion Although arthrocentesis performed better than conservative management the findings should be interpreted cautiously, and non-invasive management considered as primary measure. Still, several knowledge gaps concerning surgical methods of choice remains.

Head and neck cancer (HNC) patients suffer from a range of health-related quality of life (HRQoL) issues, but little is known about their long-term HRQoL. This study explored associations between treatment group and HRQoL at least 5 years' post-diagnosis in HNC survivors. In an international cross-sectional study, HNC survivors completed the European Organization for Research and Treatment of Cancer (EORTC) quality of life core questionnaire (EORTC-QLQ-C30) and its HNC module (EORTC-QLQ-H&N35). Meaningful HRQoL differences were examined between five treatment groups: (a) surgery, (b) radiotherapy, (c) chemo-radiotherapy, (d) radiotherapy ± chemotherapy and neck dissection and (e) any other surgery (meaning any tumour surgery that is not a neck dissection) and radiotherapy ± chemotherapy. Twenty-six sites in 11 countries enrolled 1105 survivors. They had a median time since diagnosis of 8 years, a mean age of 66 years and 71% were male. After adjusting for age, sex, tumour site and UICC stage, there was evidence for meaningful differences (10 points or more) in HRQoL between treatment groups in seven domains (Fatigue, Mouth Pain, Swallowing, Senses, Opening Mouth, Dry Mouth and Sticky Saliva). Survivors who had single-modality treatment had better or equal HRQoL in every domain compared to survivors with multimodal treatment, with the largest differences for Dry Mouth and Sticky Saliva. For Global Quality of Life, Physical and Social Functioning, Constipation, Dyspnoea and Financial Difficulties, at least some treatment groups had better outcomes compared to a general population. Our data suggest that multimodal treatment is associated with worse HRQoL in the long-term compared to single modality.

Recent literature indicates that post-COVID-19 patients suffer from a plethora of complications, including chemosensory dysfunction. However, little attention has been given to understand the interactions between chemosensory, trigeminal, and salivary dysfunctions in these patients. The aims of this study were (1) to investigate the prevalence and combinations of chemosensory, trigeminal, and salivary dysfunctions, (2) to identify the odorants/tastants that are compromised, and (3) to explore possible associations between the four dysfunctions in post-COVID-19 patients. One hundred post-COVID-19 patients and 76 healthy controls (pre-COVID-19) were included in this cross-sectional, case-controlled study. Participants' smell, taste, trigeminal, and salivary functions were assessed. The patients had a significantly higher prevalence of parosmia (80.0%), hyposmia (42.0%), anosmia (53.0%), dysgeusia (34.0%), complete ageusia (3.0%), specific ageusia (27.0%), dysesthesia (11.0%) and dry mouth (18.0%) compared to controls (0.0% for all parameters, except 27.6% for hyposmia). Complete loss of bitter taste was the most prevalent specific ageusia (66.7%) and coffee was the most common distorted smell (56.4%). Seven different combinations of dysfunction were observed in the patients, the most common being a combination of olfactory and gustatory dysfunction (48.0%). These findings indicate that post-COVID-19 patients experience a range of chemosensory, trigeminal, and salivary disturbances, occurring in various combinations.

Background Patients suffering from psychiatric disorders face many difficulties due to their condition, medications and lifestyle. Oral health and nutrition may be affected, further complicating their lives. Our aim was to provide in-depth information on oral health and nutritional factors in a small group of patients in short-term psychiatric ward. Methods Twenty-three patients (mean age 36, average medications five) were recruited during short-term hospitalization in a psychiatric ward. Inclusion criteria: anxiety, psychosis and/or depression, and use of at least one antidepressant or anxiolytic/antipsychotic drug with xerostomia as a known side effect. Subjective oral dryness was evaluated using the Shortened Xerostomia Inventory (SXI). Oral examination included Clinical Oral Dryness Score (CODS), secretion of unstimulated (UWS) and stimulated whole saliva (SWS), and evaluation of dental, gingival, and periodontal status. Self-reported complaints of oral disorders were recorded. The Oral Health Impact Profile-14 (OHIP-14) was used to explore oral health-related quality of life. Nutritional status was assessed using the Patient-Generated Subjective Global Assessment Short Form (PG-SGA-SF), and diet quality was assessed using the Mediterranean diet score (KIDMED). Results Compared to healthy controls, the patients had significantly higher SXI scores and CODS, and SWS secretion was lower. Complaints of dysgeusia and halitosis were significantly more frequent among patients. Gingivitis was more common in patients. OHIP-14 scores were much higher in the patients, and they reported significantly poorer oral and general health. Most patients lacked a regular meal pattern. Very low diet quality was observed in five patients, while improvements were needed in twelve. “ Dry mouth ” and “ No appetite, just did not feel like eating ” were the most common symptoms preventing patients from eating enough. The PG-SGA-SF symptoms component score showed a strong negative correlation with self-reported oral health, and a strong positive correlation with OHIP-14. Conclusion This relatively small group of patients in short-term psychiatric ward had both reduced oral health and poor oral health-related quality of life. Furthermore, their nutritional intake was affected by their oral health problems. Although larger groups need to be studied, these findings indicate that oral health and nutrition should be evaluated and adjusted in these patients to improve their overall care.

Sjögren’s syndrome is an autoimmune rheumatic disease characterized by inflammation of the salivary and lacrimal glands, often manifesting as dry mouth and dry eyes. To simplify diagnostics of primary Sjögren’s syndrome (pSS), a non-invasive marker is needed. The aim of the study was to compare the RNA content of salivary extracellular vesicles (EVs) between patients with pSS and healthy controls using microarray technology. Stimulated whole saliva was collected from 11 pSS patients and 11 age-matched controls. EV-RNA was isolated from the saliva samples using a Qiagen exoRNeasy Midi Kit and analyzed using Affymetrix Clariom D™ microarrays. A one-way ANOVA test was used to compare the mean signal values of each transcript between the two groups. A total of 9307 transcripts, coding and non-coding RNA, were detected in all samples. Of these transcripts, 1475 showed statistically significant differential abundance between the pSS and the control groups, generating two distinct EV-RNA patterns. In particular, tRNAs were downregulated in pSS patients, with the transcript tRNA-Ile-AAT-2-1 showing a 2-fold difference, and a promise as a potential biomarker candidate. This study therein demonstrates the potential for using salivary EV-RNA in pSS diagnostics.

This study investigates the stability and cytotoxicity of biopolymer-coated liposomes for use in the oral cavity. Liposomes (3 mM and 6 mM) were prepared by the thin film method and hydrated with phosphate buffer (PB) or glycerol phosphate buffer (G-PB). For coating, liposomes were added to a biopolymer solution of opposite charge. Particle stability was evaluated by measuring the size, polydispersity index, and zeta potential for up to 60 weeks. In vitro interaction of fluorescent-labelled biopolymer-coated liposomes and dysplastic oral keratinocytes was analyzed by confocal microscopy. Potential cytotoxicity was assessed in dysplastic oral keratinocytes by cell proliferation and cell viability. All three biopolymers showed good coating abilities for both concentrations and hydration media. The alginate coated liposomes in PB, 3 mM chitosan-coated liposomes in PB, and chitosan-coated liposomes in G-PB were stable for up to 60 weeks. In vitro studies demonstrated low cytotoxicity for all coated liposomes and non-specific cellular uptake of biopolymer-coated liposomes, independent of biopolymer, surface charge, lipid concentration and hydration media. All three formulations demonstrated low cytotoxicity and were considered safe. Alginate- and chitosan-coated liposomes demonstrated good stability over time and may be promising agents for use in the oral cavity and should be investigated further.

Erythema multiforme following a respiratory tract infection – a rare diagnosis A 13-year-old girl with a recent respiratory infection developed pronounced oral ulcerations and was diagnosed with erythema multiforme (EM) minor. The infection was attributed to Mycoplasma pneumoniae, human bocavirus, and Epstein-Barr virus (EBV), as indicated by IgG seropositivity. Clinically, no cutaneous involvement was observed. The condition was managed with topical corticosteroid therapy and nutritional support. The patient demonstrated marked clinical remission within four days. This case highlights the importance of considering EM as a potential cause of severe oral lesions in children following respiratory infections and illustrates that localized treatment can be both effective and sufficient in such presentations.

Background: Polypharmacy and inappropriate drug use are associated with adverse health outcomes in older people. Collaborative interventions between geriatricians and general practitioners have demonstrated effectiveness in improving clinical outcomes for complex medication regimens in home-dwelling patients. Since 2012, Norwegian municipalities have established municipal in-patient acute care (MipAC) units, designed to contribute towards reducing the number of hospital admissions. These units predominantly serve older people who typically benefit from multidisciplinary approaches. The primary objective of this study is to evaluate the effect of cooperative medication reviews conducted by MipAC physicians, supervised by geriatricians, and in collaboration with general practitioners, on health-related quality of life and clinical outcomes in MipAC patients ≥ 70 years with polypharmacy. Additionally, the study aims to assess the carbon footprint of the intervention. Methods: This is a randomized, single-blind, controlled superiority trial with 16 weeks follow-up. Participants will be randomly assigned to either the control group, receiving usual care at the MipAC unit, or to the intervention group which in addition receive clinical medication reviews that go beyond what is considered usual care. The medication reviews will evaluate medication appropriateness using a structured but individualized framework, and the physicians will receive supervision from geriatricians. Following the clinical medication reviews, the MipAC physicians will arrange telephone meetings with the participants’ general practitioners to combine their assessments in a joint medication review. The primary outcome is health-related quality of life as measured by the 15D instrument. Secondary outcomes include physical and cognitive functioning, oral health, falls, admissions to healthcare facilities, and mortality. Discussion: This study aims to identify potential clinical benefits of collaborative, clinical medication reviews within community-level MipAC units for older patients with polypharmacy. The results may offer valuable insights into optimizing patient care in comparable municipal healthcare settings. Trial registration: The study was registered prospectively on ClinicalTrials.gov 30.08.2023 with identifier NCT06020391.

Oral symptom and problems are highly prevalent in patient with advanced cancer, and xerostomia is reported in 84% of the cases. Between 30% - 50% of the patients develop oral infections, which most common infection is oral candidiasis. Despite the symptoms negative impact on quality of life, oral symptoms and problems are not systematically assessed or treated in palliative care. The evidence supporting the effectiveness of mouth rinses in alleviating oral discomfort is limited. Additionally, knowledge of oral fungal species and their susceptibility to antifungal agents are spares. The overall aim of this thesis was to investigate oral symptoms and problems in patients with limited life expectancy receiving inpatient palliative care due to advanced cancer. In study 1 we performed an interventions study where we investigated the effect of a mouth rinse based on the herb Salvia Officinalis (SO) compared to standard care using normal saline in 88 patients with advanced cancer. Both groups showed statistically significant improvement in their overall oral symptoms and SO was as effective as standard care. In study 2 we conducted a microbiological analysis of oral swab tests from 45 patients taken during study 1. Seven different Candida species were detected, and nine patients had mixed infection. The susceptibility to antifungal agents was high. In Study 3, we observed the symptom dry mouth in two different versions of a symptom assessment questionnaire. The symptom dry mouth was included in one version, while in a revised version, patients were required to report dry mouth as an “Other problem”. Dry mouth was the third most reported symptom when prompted in a symptom list, whereas the symptom was significantly less reported in the revised version. This thesis highlights the presence of oral symptoms and problems in patients with advanced cancer and has shown that SO mouth rinse was as good as normal saline in an oral care procedure. Furthermore, there is a diversity of Candida species and mixed oral infections occurs. Finally, systematic assessment is crucial to identify the present of different oral symptoms.
